Medicaid's January 2027 Double Deadline
An Implementation Guide For Work Requirements And Six-Month Renewals
A member who works 90 hours a month can still lose Medicaid in 2027 if the proof doesn't reach the state in time. Most 2023–24 unwinding losses happened that way. From January 1, 2027, expansion adults must meet a new community engagement requirement and renew their coverage every six months instead of once a year. Every extra renewal is another form that can arrive late. In my view, the states that keep eligible people covered will be the ones that check the data they already hold before they ask anyone for proof.

The Problem
What changed Signed on July 4, 2025, H.R. 1 links Medicaid for many expansion adults to 80 hours a month of work, school or community service. It also halves the time between eligibility checks.
What breaks Roughly 24 million people lost Medicaid in the 2023–24 unwinding. For between 66% and 74% of them, the cause was a late form or an unanswered notice rather than a finding that they no longer qualified. Each new rule adds forms to that same process.
What it costs CMS itself expects about 2.3 million people to lose coverage in 2027 because of the work requirement. Some will be people who did the hours and couldn't prove it. Each state decides how many, through the data it checks first and the plain wording of the notices it sends to members.
